BACKGROUND: In the nasal cavity, nitric oxide (NO) is involved in many physiological functions, including antibacterial and antiviral activity, promotion of nasal mucociliary clearance, and regulation of blood vessel expansion in the nasal mucosa. We investigated the distribution of NO concentration in the nasal cavity of healthy individuals during breathing. METHODS: A three-dimensional numerical model of the nasal airway, including the bilateral maxillary sinuses, was created to simulate NO distribution in the nasal cavity during normal breathing. The effect of different nasal airflow velocities and NO concentrations in the maxillary sinus on NO distribution in the nasal cavity was evaluated. The NO concentration in the nasal exhalation of 50 healthy people in Dalian was measured using an NO analyzer, and the growth rate of the NO concentration in the nasal cavity was measured under breath-holding conditions. RESULTS: The distribution of NO concentration in the nasal cavity of healthy people during breathing was obtained from numerical simulation results. Lower the airflow rate, higher was the NO concentration and greater was the diffusion range in the nasal cavity. The NO concentration in the nasal cavity increased with an increase in its concentration in the maxillary sinus, indicating a linear relationship. The NO concentration in the nasal exhalation of healthy people in Dalian and the growth rate of the NO concentration in the nasal cavity under breath-holding conditions were obtained through experiments. The numerical results correspond with the experimental results. CONCLUSIONS: The NO entered the nasal cavity mainly by diffusion and followed the convection flow of the respiratory air in the nasal cavity. NO concentration in the nasal cavity was related to the respiratory airflow velocity and NO concentration in the maxillary sinus. During inspiration, NO was present only in the nasal airway posterior to the maxillary sinus ostium, whereas during exhalation, the exhaled NO diffusely distributed throughout the nasal cavity.

Objetive: To compare the stresses and deformations generated on the surrounding bone of the zygomatic implants when using an intra sinusal and extra-maxillary approach, through the finite element method. Material and Methods: Computer aided designs (CADs) were constructed using SolidWorks Software of a skull with bone resorption to be rehabilitated through a fixed hybrid prosthesis using two zygomatic and two conventional straight implants. For the boundary conditions (load conditions), symmetry in the sagittal plane was assumed and that all the materials were isotropic, homogeneous and linearly elastic. Two zygomatic implantation techniques were simulated: intra sinusal (Is) and extra maxillary (Em). Vertical and lateral loads of 150 N and 50 N were applied to the finite element models to obtain Von Mises equivalent stress and strain (displacement). Results: The average measurement of the Von Mises stress (MPa) recorded were as follows: Approach of the implant body (Is: 0.24- Em: 0.28,) effort of implant body with vertical load: Is: 0.69 - Em: 0.96; effort of peri-implant surface under horizontal load: Is: 2.11 - Em: 0.94. Average displacement under vertical load of peri-implant surface Is: 0.35 - Em: 0.40, and of implant body Is: 1.34 - Em: 2.04. Average total deformation in approach Is: 2.23 mm - Em: 0.80mm, and average total deformation in the implant body under horizontal load was Is: 0.14 - Em: 0.21. Conclusion: The results of this study indicate that despite the differences that occurred in both stress and strain (displacement) between the intra-sinus and extra-maxillary approaches, the static strength of the bone, which is approximately 150 MPa in tension and 250 MPa in compression was not exceeded. Considering the limitations of finite element analysis, there seems to be no biomechanical reason to choose one approach over the other.


Objetivo: Comparar por el método de elementos finitos los esfuerzos y deformaciones generados sobre el hueso circundante de implantes cigomáticos tratados con un abordaje intra sinusal y extra maxilar. Material y Métodos: Se construyeron los diseños asistidos por computadora (CAD) utilizando el Software SolidWorks de un cráneo con una reabsorción ósea para ser rehabilitado, a través de una prótesis híbrida fija, mediante dos implantes cigomáticos y dos rectos convencionales. Para las condiciones de frontera (condiciones de carga) se asumió simetría en el plano sagital y que todos los materiales eran isotrópicos, homogéneos y linealmente elásticos. Se simularon dos técnicas de implantación cigomática: una intra sinusal (Is) y otra extra maxilar (Em). Se aplicaron cargas verticales y laterales de 150 N y 50 N a los modelos de elementos finitos para obtener el esfuerzo equivalente de Von mises y la deformación (desplazamiento). Resultados: La medición promedio del esfuerzo de Von Mises (MPa) registró: abordaje del cuerpo de implante (Is: 0.24-Em: 0.28) esfuerzo del cuerpo de implante con carga vertical: (Is:0.69 ­ Em: 0.96); esfuerzo de la superficie peri implantar ante carga horizontal (lateral):( Is:2.11 ­ Em:0.94). Desplazamiento promedio ante carga vertical de la superficie peri implantar (Is:0.35 ­ Em:0.40) y del cuerpo del implante (Is:1.34 ­ Em:2.04). Deformación total promedio en mm en abordaje (Is: 2.23 ­ Em:0.80) y deformación total promedio en el cuerpo del implante ante carga horizontal (Is:0.14 ­ Em:0.21). Conclusión: Los resultados de este estudio indican que a pesar de las diferencias que se presentaron tanto en el esfuerzo como en la deformación (desplazamiento) entre los abordajes intra sinusal y extra maxilar, la resistencia estática del hueso, que es de aproximadamente 150 MPa en tensión y 250 MPa en compresión no se superó. Considerando las limitaciones de los AEF, parece no haber razones biomecánicas para elegir uno u otro enfoque.

Functional endoscopic sinus surgery (FESS) is performed to treat sinusitis when treatment with medication fails. In the present study, three different virtual maxillary sinus endoscopic surgeries were performed on a realistic 3-D computational model of the nasal cavity of an adult male under the supervision of a specialist. They included only uncinectomy, uncinectomy + 8mm Middle Meatal Antrostomy (MMA) and uncinectomy + 18 mm MMA. Simulations were performed for two human activity respiratory rates, including rest and moderate activities, and effects of different surgeries and respiratory rates on maxillary sinus were investigated. It was found that after endoscopic sinus surgery, the volume of air entering the maxillary sinus increased significantly, and as the size of the MMA increased, or the breathing condition changed from rest to moderate activity, this volume of air increased. For the rest condition, on average for both nasal passages, for uncinectomy +8 mm MMA, around 15 % of the inhaled flow and 7 % of the exhaled flow enter the maxillary sinuses. For uncinectomy +18 mm MMA, these values are 24 % and 14 %, respectively. As human activity increases, a lower portion of inhaled and exhaled air enters the maxillary sinuses. For the moderate activity condition, on average for both nasal passages, for uncinectomy +8 mm MMA, around 11 % of the inhaled flow and 6 % of the exhaled flow rate enters the maxillary sinus. For uncinectomy +18 mm MMA, these values are 16 % and 8%, respectively. Comparing the steady and unsteady simulation results showed that the quasi-steady flow assumption could predict the flow in the maxillary sinus and the volume of air entering the sinuses, almost at any moment of respiration, with acceptable accuracy.

This study is aimed at determining the optimal sinus augmentation approach considering the poor bone condition in the zone of atrophic posterior maxilla. A series of simplified maxillary segment models varying in residual bone height (RBH) and bone quality were established. A 10 mm standard implant combined with two types of maxillary sinus augmentation methods was applied with the RBH, which was less than 10 mm in the maxilla. The maximal equivalent von Mises (EQV) stress in residual bone was evaluated. Bone quality had an enormous impact on the stress magnitude of supporting bone. Applying sinus augmentation combined with grafts was suitable for stress distribution, and high-stiffness graft performed better than low-stiffness one. For 7 mm and 5 mm atrophic maxilla, nongrafted maxillary sinus augmentation was feasible in D3 bone. Poor bone quality was a negative factor for the implant in the region of atrophic posterior maxilla, which could be improved by grafts. Meanwhile, the choice of maxillary sinus augmentation approaches should be determined by the RBH and quality.

BACKGROUND: The question of the "true" function of the maxillary sinus and the paranasal sinuses (PS) has been a controversial issue in the literature for decades, leading to many discussions and speculations. OBJECTIVE: This review briefly summarizes various theories on the possible physiology and functions of the maxillary sinus/PS that have been discussed over the centuries. MATERIALS AND METHODS: A literature search was conducted in PubMed using a combination of the search terms "physiology," "function," "maxillary sinus," and "paranasal sinuses." RESULTS: Current and scientifically evidenced theories are described. "Sinusology" is the science of the PS. The maxillary sinuses might simply serve to improve the respiratory function of the nose. A flow of inspiratory air does not occur. The maxillary sinuses are decisively involved in the production of nitrogen monoxide (NO) and thus in supporting the immune defense of the nasal cavity. The mucosa of the maxillary sinus continuously synthesizes NO and serves as a reservoir of NO. Other important functions are protection of the orbit and the brain in case of skull fractures, as well as weight reduction of the skull. CONCLUSION: The various theories about the function of the PS still raise many questions and their true function is yet not fully understood. Possible functions of the maxillary sinuses are local immune defense through the production of NO. The PS serve as a crumple zone for vital cerebral structures in the context of craniocerebral traumas.

ABSTRACT: The objective of this study is to discuss and analyze whether extrusion of endodontic material constitutes avoidable intercurrence, discussing the clinical, ethical and legal implications. Patient LSR, 31 years old, female, attended a dental consultation complaining of pain in the second left maxillary premolar (tooth 25). Radiographically, a single root canal and thickening of the periodontal ligament associated with extravasation of 8 mm of gutta percha to the maxillary sinus were observed. The first endodontic session aimed to performing the desobturation, root canal preparation and intracanal medication placement. The root canal obturation was performed in the second session. Was carried out an apical surgery that removed 2 mm from the root apex and also performed the covered with a collagen membrane. A 22 months follow-up revealed a tooth function, absence of painful symptomatology or infection, and radiographically normal periodontal ligament and bone neoformation. The second single-root premolar is the type of premolar with less distance with the floor of the maxillary sinus. In this case, the extrusion of the obturator material occurred due to the superinstrumentation of the root canal associated with the proximity of the root with the membrane of the maxillary sinus. From the ethical and legal point of view, the patient has the right to be informed about any intercurrences that may arise during or after the treatment, and the informed consent form is essencial. This document will allow the patient to make a decision about performing an endodontic treatment in which the risk of an accident or complication is imminent or that treatment failure is already expected. It is important that professionals make appropriate diagnosis and treatment plan for each case, since this conduct may avoid clinical intercurrences. In addition, if the intercurrences occur, the patient should be advised of how to proceed.


RESUMEN: El objetivo de este estudio fue discutir y analizar si la extrusión de material endodóntico constituye una intercurrencia evitable, discutiendo las implicaciones clínicas, éticas y legales. Paciente LSR, de 31 años de edad, mujer, asistió a una consulta dental quejándose de dolor en el segundo premolar maxilar izquierdo (diente 25). Radiográficamente, se observó un solo conducto radicular y engrosamiento del ligamento periodontal asociado con la extravasación de 8 mm de gutapercha al seno maxilar. La primera sesión de endodoncia tuvo como objetivo realizar la desobturación, la preparación del conducto radicular y la colocación de medicación intracanal. La obturación del conducto radicular se realizó en la segunda sesión. Se llevó a cabo una cirugía apical que extrajo 2 mm del ápice de la raíz y también se realizó el recubrimiento con una membrana de colágeno. Un seguimiento de 22 meses reveló función dental, ausencia de sintomatología dolorosa o infección y ligamento periodontal radiográficamente normal y neoformación ósea. El segundo premolar de una sola raíz es el tipo de premolar con menos distancia con el piso del seno maxilar. En este caso, la extrusión del material obturador se produjo debido a la superinstrumentación del conducto radicular asociada con la proximidad de la raíz con la membrana del seno maxilar. Desde el punto de vista ético y legal, el paciente tiene derecho a ser informado acerca de cualquier inter-ocurrencia que pueda surgir durante o después del tratamiento, y el formulario de consentimiento informado es esencial. Este documento le permitirá al paciente tomar una decisión sobre la realización de un tratamiento de endodoncia en el que el riesgo de un accidente o complicación sea inminente o de que ya se espera un fracaso del tratamiento. Es importante que los profesionales realicen un diagnóstico y un plan de tratamiento adecuados para cada caso, ya que esta conducta puede evitar las intercurrencias clínicas. Además, si se producen intercurrencias, se debe informar al paciente sobre cómo proceder.

OBJECTIVES: The maxillary sinus is of paramount importance for otolaryngologists, rhinologists, oral and maxillofacial surgeons, head and neck and dental and maxillofacial radiologists. A comprehensive review article concerning the physiology, development and imaging anatomy was undertaken. METHODS: Relevant literature pertaining to the physiology of the sinonasal cavity, development of the paranasal sinuses and imaging anatomy of the maxilla and maxillary sinus from 2000 to 2019 was reviewed. Emphasis was placed on literature from the last 5 years. RESULTS: Extensive recent research using imaging has provided new insights into the development of the maxillary sinus, the other paranasal sinuses and the midface. The fundamental physiological concept of mucociliary clearance and its role in sinus health is emphasized. The paranasal sinuses are an integral part of a common mucosal organ formed by the upper and lower airway.An in-depth understanding of the soft-tissue and neurovascular relationships of the maxillary sinus to the deep fascial spaces and branches of the trigeminal nerve and external carotid artery respectively is required to evaluate and report imaging involving the maxillary sinus.Sinusitis of rhinogenic, rather than odontogenic origin, originates from nasal inflammation followed by anterior ethmoid disease and secondary obstruction of the ostiomeatal unit. The role of anatomical variants that predispose to this pattern of disease is discussed in detail with illustrative examples.The maxillary sinus is intimately related to the roots of the posterior maxillary teeth; the high frequency of mucosal disease and sinusitis of odontogenic aetiology is now well recognized. In addition, an understanding of the anatomy of the alveolar process, morphology of the alveolar recess of the maxillary sinus and neurovascular supply are essential both for deliberate surgical intervention of the sinus and complications related to oral surgical procedures. CONCLUSIONS: An understanding of the fundamental principles of the development, physiology, anatomy and relationships of the maxillary sinus as depicted by multi-modality imaging is essential for radiologists reporting imaging involving the paranasal sinuses and midface.

The poly D,L-lactide-co-glycolic acid (PLGA) is a copolymer used in many therapeutic devices for its high rates of biodegradability and biocompatibility. The principal aim of the research was to evaluate the new bone formation, after 16 (T1) and 28 weeks (T2), in sheep maxillary sinus lift in vivo model using PLGA.Computerized tomography analysis, X-ray microanalysis, and scanning electron microscope analysis of secondary electrons (SE) and the backscattered electrons (BSE) of the samples were detected.After 28 weeks, the computed tomography analysis showed a 22% increase of UH density in the grafting areas. The X-ray microanalysis of the samples showed calcium and phosphorus increase at T1 and T2 follow-up period and the carbon and oxygen concentration decrease. The SE evaluation showed a rapid superficial resorption of the biomaterials at T1 and a completely bone reorganization of biomaterial at T2. The BSE analysis confirmed the SE data and showed the direct and intimate contact between bone and PLGA with a higher calcification in T2 compared to T1.Certainly, still other experiments and a larger number of samples will be necessary to be analyzed to determine the behavior of the PLGA in the bone regeneration; however, the PLGA used in maxillary sinus lift animal model, seem to promote new bone formation that continues increase at 28 weeks after grafting.

Injectable bone cement (IBC) such as those based on methacrylates and hydraulic calcium phosphate and calcium sulfate-based cements have been used extensively for filling bone defects with acceptable clinical outcomes. There is a need however for novel IBC materials that can address some of the inherent limitations of currently available formulations to widen the clinical application of IBC. In this study, we characterized a novel hydraulic IBC formulation consisting of bioactive strontium-doped hardystonite (Sr-HT) ceramic microparticles and sodium dihydrogen phosphate, herein named Sr-HT phosphate cement (SPC). The resultant cement is comprised of two distinct amorphous phases with embedded partially reacted crystalline reactants. The novel SPC formulation possesses a unique combination of physicochemical properties suitable for use as an IBC, and demonstrates in vitro cytocompatibility when seeded with primary human osteoblasts. In vivo injection of SPC into rabbit sinus defects show minor new bone formation at the SPC periphery, similar to those exhibited in sinus defects filled with a clinically available calcium phosphate cement. The current SPC formulation presented in this paper shows promise as a clinically applicable IBC which can be further enhanced with additives.

RESUMEN

Silent sinus syndrome (SSS) is a rare disorder involving bony remodeling of the maxillary sinuses, including collapse of the orbital floor. It is typically unilateral and seen in the setting of chronic maxillary sinusitis. Patients present with enophthalmos and hypoglobus occasionally accompanied by diplopia. The condition is generally diagnosed with CT imaging and treated surgically. While SSS is most often reported as very slowly progressive, we report a patient with evidence of significant remodeling of the maxillary sinus over a period of 5.5 months.

OBJECTIVE: This study aims to expand knowledge of Concha bullosa in a prehistoric population. MATERIALS: The skull of an adult female from the Bronze Age archeological site ofCastellón Alto (Granada, Spain), dated between 1900 and 1600 BCE. METHODS: The diagnosis of middle turbinateconcha bullosa was based on macroscopic, radiographic, and computed tomography studies. RESULTS: This is one of the oldest findings ofconcha bullosa in Europe. Despite the possible obstruction of the osteomeatal complex by the concha bullosa in this individual, no signs of sinusitis were observed in maxillary sinuses. CONCLUSIONS: The maxillary sinuses of this female showed no signs of sinusitis related to theconcha bullosa. SIGNIFICANCE: This study provides new evidence on the chronological limits of this condition. The particular funeral rites of these populations have preserved the majority of skeletons at this site in an excellent condition. LIMITATIONS: The identification ofconcha bullosa continues to be rare in ancient populations, in part due to the poor preservation of nasal bones in archeological collections. Suggestions for further research: Detailed study of the nasal cavity of adequately preserved individuals can be expected to identify further cases of concha bullosa in ancient populations.

SUMMARY: The objective of this study was to investigate the prevalence and location of maxillary sinus septa prior to dental implant therapy in different age groups by cone beam computed tomography (CBCT) and identify the most helpful orthogonal plane for this purpose. CBCT scans of 200 patients were selected. The final sample was divided into four groups according to age: (1) 30-39 y.o.; (2) 40-49 y.o.; (3) 50-59 y.o.; (4) +60 y.o. Three calibrated examiners assessed the presence and location (anterior, middle, posterior) of sinus septa using all orthogonal slices. The prevalence of septa per region and plane was compared by chi-square test. Among 359 maxillary sinuses evaluated, it was observed at least one septum in 163 sinuses (45.4 %). Overall, the anterior region was the most prevalent. However, when the age groups were considered, the anterior region was only the most prevalent in groups 2 and 3. The middle and posterior regions were more prevalent in groups 4 and 1, respectively. In conclusion, the prevalence of maxillary sinus septa in patients who were candidate to dental implant therapy was notably high. It was observed that the anterior region of the sinus was the most prevalent and the axial slice was the most representative.


RESUMEN: El objetivo de este estudio fue investigar la prevalencia y localización del septo del seno maxilar antes del tratamiento con implantes dentales en diferentes grupos etarios, por medio de la Tomografía Computarizada de Haz Cónico (TCHC). Fueron seleccionadas TCHC de 200 pacientes. La muestra final fue dividida en cuatro grupos de acuerdo con la edad: (1) 30-39 años; (2) 40-49 años; (3) 50-59 años y (4) +60 años. Tres examinadores calibrados evaluaron la presencia y localización (anterior, media o posterior) del septo del seno maxilar usando todas las secciones ortogonales. La prevalencia del septo por región y plano fue comparada mediante el test x-cuadrado. Entre los 359 senos maxilares evaluados, fue observado por lo menos un septo en 163 senos (45,4 %). Generalmente la región anterior fue la más prevalente. Sin embargo, cuando fueron considerados los grupos etarios, solamente la región anterior fue la más prevalente en los grupos 2 y 3. Las regiones media y posterior fueron las más prevalente en los grupos 4 y 1, respectivamente. La prevalencia del septo del seno maxilar en pacientes que son candidatos a tratamiento con implantes dentales fue notablemente alta. Se observó que la región anterior fue la más prevalente siendo las secciones axiales las más representativas.

BACKGROUND: To investigate the feasibility, safety and stability of current interventions for moving teeth through the maxillary sinus (MTTMS) by performing a systematic review of the literature. METHODS: The electronic databases PubMed, Embase, CENTRAL, Web of Science, CBM, CNKI and SIGLE were searched without a language restriction. The primary outcomes were parameters related to orthodontic treatment, including orthodontic protocols, magnitude of forces, type of tooth movement, duration and rate of tooth movement, and remolding of alveolar bone and the maxillary sinus floor. The secondary outcomes were safety and stability, including root resorption, perforation of the sinus floor, loss of pulp vitality and periodontal health and relapse. RESULTS: Nine case reports with 25 teeth were included and systematically analyzed. Fifty to two hundred g of force was applied to move teeth through the maxillary sinus. Bodily movement was accomplished, but initial tipping was observed in 7 cases. The rate was 0.6-0.7 mm/month for molar intrusion and 0.16-1.17 and 0.05-0.16 mm/month for mesial-distal movement of premolars and molars, respectively. Bone formation and remolding of the sinus floor occurred in 7 cases. Root resorption within 6 to 30 months was observed in 3 cases, while no cases of perforation of the sinus floor, loss of pulp vitality, periodontal health impairment or relapse were reported. CONCLUSIONS: At the present stage, no evidence-based protocol could be recommended to guide MTTMS. The empirical application of constant and light to moderate forces (by TAD, segment and multibrackets) to slowly move teeth through or into the maxillary sinus in adults appears to be practical and secure. Bodily movement was accomplished, but teeth appear to be easily tipped initially, potentially resulting in root resorption. However, this conclusion should be interpreted with caution as the currently available evidence is based on only a few case reports or case series and longitudinal or controlled studies are lacking in this area.

The purpose of this study was to evaluate the airway volume changes associated with rapid maxillary expansion (RME) protocols using different activation rates. A sample of forty RME treated cases was classified into two groups according to the rate of RME activation (Group A: 0.8mm per day and Group B: 0.5mm per day). Three-dimensional images were obtained for each case before and three months after expansion. Dolphin imaging software was used to identify landmarks and calculate airway volumes. Intraclass correlation coefficient was used to confirm reliability and Wilcoxon signed rank tests were used for comparison between the initial and final measurements within each group and between groups. Significant increase in the nasal cavity volume for both groups was observed (P<0.0001 and P=0.001 for groups A and B respectively). The increase in nasopharynx volume was significant in the group with a more rapid activation rate (P=0.0006). Significant differences between the two groups in post-treatment changes were detected in the nasal cavity volume (P<0.0001), nasopharynx volume (P=0.0035), and soft palate area (P=0.0081). A more rapid activation rate results in a higher volume increase for the nasal cavity and nasopharynx than a slower activation rate.

BACKGROUND The aim of this study was to observe the pneumatization degree and adjacent structure of the posterior superior maxillary sinus (PSMS) and its effect on nasal cavity morphology. MATERIAL AND METHODS The study included a total of 103 cases whose paranasal sinus CT scans had been analyzed. The pneumatization of the PSMS and its relationship with posterior ethmoid sinus (PEs) and sphenoid sinus (SS) were observed. The effects of the pneumatization of PSMS on nasal cavity width and morphology also were evaluated. RESULTS 1) The PSMS was adjacent to orbit or middle nasal meatus (MNM) as type I in 5.82% of cases. The PSMS was adjacent to the orbit and superior nasal meatus (SNM) as follows: the superior part of medial wall of maxillary sinus (MMS) was not abutting on PEs as type II (4.35%) and abutting on PEs as type III (85.9%). If the type III was not accompanied by MMS shift toward medial it was identified as type IIIa (33.50%), and if it was accompanied MMS shift toward medial, it was identified as type IIIb (45.63%). The ethmomaxillary sinus (EMS) was identified as type IIIc (6.80%). The PSMS directly abutted on the SS as type IV in (3.88%). 2) The higher the degree of the pneumatization of PSMS was, the narrower the width of the upper part of the posterior nasal cavity (p<0.05 respectively). CONCLUSIONS The relationship of PSMS with the orbit, SNM, PEs, and SS should be identified pre-operation; it is important for safety and complete removal of retromaxillary lesions during endoscopic sinus surgery. The pneumatization degree of PSMS also should be considered as it can influence the morphology of posterior nasal cavity.

Numerous grafting materials have been used to augment the maxillary sinus floor for long-term stability and success for implant-supported prosthesis. To enhance bone formation, adjunctive blood-born growth factor sources have gained popularity during the recent years. The present study compared the use of platelet-rich fibrin (PRF) and bovine-autogenous bone mixture for maxillary sinus floor elevation. A split-face model was used to apply 2 different filling materials for maxillary sinus floor elevation in 22 healthy adult sheep. In group 1, bovine and autogenous bone mixture; and in group 2, PRF was used. The animals were killed at 3, 6, and 9 months. Histologic and histomorphologic examinations revealed new bone formation in group 1 at the third and sixth months. In group 2, new bone formation was observed only at the sixth month, and residual PRF remnants were identified. At the ninth month, host bone and new bone could not be distinguished from each other in group 1, and bone formation was found to be proceeding in group 2. PRF remnants still existed at the ninth month. In conclusion, bovine bone and autogenous bone mixture is superior to PRF as a grafting material in sinus-lifting procedures.

The aim of this study was to analyse the posterior maxillary sinus based on its extension into the alveolar process and to provide a simple clinical classification. A retrospective cohort study was conducted in CT scans of 200 dentate and 200 edentulous patients (100 women and 100 men, respectively). After manual placement of 12 reference points morphometric analysis was performed and sinus depth, residual alveolar ridge height (RH) and the sinus opening angle were calculated. Sinuses were classified according to the quartiles of sinus depth: class I (above the hard palate), class II (0-6 mm below the hard palate) and class III (>6 mm below the hard palate). Sinus depth was found to be a reliable anatomical landmark and did not vary significantly between gender (p = 0.8940) or dentition groups (p = 0.9723). Alveolar height varied significantly between sinus classes (p < 2 × 10-16) and dentition groups (p < 2 × 10-16) but not between genders (p = 0.5178). The sinus opening angle was significantly different between sinus classes (p < 2.2 × 10-16) but not between gender or dentition groups. We propose a novel classification built upon the quartiles of sinus depth, dividing the sinuses into three classes. Our classification is the first one that represents the anatomy of the patient independent of gender and dentition.

Previous research has suggested that the maxillary sinuses may act as "zones of accommodation" for the nasal region, minimizing the impact of climatic-related changes in nasal cavity breadth on surrounding skeletal structures. However, a recent study among modern human crania has identified that, in addition to nasal cavity breadth, sinus morphology also tracks lateral facial form, especially anterior-posterior positioning of the zygomatics. Here, we expand upon this previous study to further investigate these covariation patterns by employing three samples with distinct combinations of nasal and zygomatic morphologies: Northern Asians (n = 28); sub-Saharan Africans (n = 30); and Europeans (n = 29). For each cranium, 30 landmarks were digitized from CT-rendered models and subsequently assigned to either a midfacial or maxillary sinus "block." Two block partial least squares (2B-PLS) analyses indicate that sinus morphology primarily reflects superior-inferior dimensions of the midface, rather than either nasal cavity breadth or zygomatic position. Specifically, individuals with relatively tall midfacial skeletons exhibit more inferiorly and laterally expanded sinuses compared to those with shorter midfaces. Further, separate across-group and within-group 2B-PLS analyses indicate that regional differences between samples primarily build upon a common pattern of midfacial and sinus covariation already present within each regional group. Allometry, while present, only explains a small portion of the midface-sinus covariation pattern. We conclude that previous findings of larger maxillary sinuses among cold-adapted individuals are not predominantly due to possession of relatively narrow nasal cavities, but to greater maxillary and zygomatic heights. The aim of this study is to compare the bone formation in maxillary sinus lift with an autogenous bone graft in histological evaluation at 2 or 6 months. A comparative study was designed where 10 patients with missing teeth bilaterally in the posterior zone of the maxilla were selected. Patients received a particulate autogenous bone graft under the same surgical conditions, selecting a site to collect a biopsy and histological study at two months and another at six months postoperatively. Histomorphometry was performed and were used Kolmogorov-Smirnov test, student's t-test and Spearman's correlation coefficient, considering a value of p<0.05. Differences were observed in inflammatory infiltrate and vascularization characteristics; however, the group analyzed at two months presented 38.12% ± 6.64 % of mineralized tissue, whereas the group studied at 6 months presented an average of 38.45 ± 9.27 %. There were no statistical differences between the groups. It is concluded that the bone formation may be similar in intrasinus particulate autogenous bone grafts in evaluations at two or six months; under these conditions, early installation of implants is viable.


El objetivo de este estudio es comparar la formación ósea en técnicas de elevación de seno maxilar utilizando injerto óseo autógeno en evaluaciónhistológica a las 2 y 6 meses. Un estudio comparativo fue diseñado donde 10 pacientes con dientes perdidos de forma bilateral en el sector posterior de maxila fueron incluidos. Los pacientes recibieron injerto óseo particulado bajo las mismas condiciones quirúrgicas, seleccionando el sitio para tomar la biopsia y el análisis histológico respectivo a los 2 meses en un lado y 6 meses en el lado contralateral. La histomorfometría se realizó y fueron utilizadas la prueba de Kolmogorov-Smirnov, la prueba t de student y la prueba de Spearman, considerando un valor de p<0,05. Fueron observadas diferencias en infiltrado inflamatorio y características de vascularización; sin embargo, los grupos analizados a los dos meses presentaron 38.12% ± 6.64% de tejido mineralizado, mientras que los grupos de estudio a los 6 meses presentaron en promedio 38.45 ± 9.27%. No se observo diferencias significativas entre los grupos. Se concluye que la formación ósea puede ser similar en elevaciones de seno maxila realizada con hueso autógeno particulado a los dos o seis meses; en estas condiciones, la instalación temprana de implantes puede ser viable.

PURPOSE: Clarification of the protocol for using short implants is required to enable widespread use of short implants as an available treatment option. The purpose of this study was to investigate the influences of implant length and diameter, bicortical anchorage, and sinus augmentation on peri-implant cortical bone stress by three-dimensional finite element analysis. MATERIALS AND METHODS: For bone models with bone quantity A and C in the maxillary molar region, three-dimensional finite element analysis was performed using different lengths and diameters of implant computer-aided design models, and the degree of maximum principal stress distribution for each model was calculated. RESULTS: For bone quantity A models, the degree of stress distribution of the 4-mm-diameter, 6-mm-length implant was the greatest. For bone quantity C models, the degree of stress distribution of the 5-mm-diameter, 6-mm-length implant with bicortical anchorage was much smaller than that for the 4-mm-diameter, 13-mm-length implant with sinus augmentation. CONCLUSION: The results of this study suggest that 6-mm-length implants should be selected in cases with bone quantity C where the bone width permits increasing implant diameter from 4 mm to 5 mm.
